Corona Virus Information

coronavirus is a type of virus. As a group, coronaviruses are common across the world. Typical symptoms of coronavirus include fever, loss or change to your sense of smell or taste or a cough that may progress to a severe pneumonia causing shortness of breath and breathing difficulties.


TYPE 1 OPT OUT PREFERENCE

The data held in your GP medical records is shared with other healthcare professionals for the purposes of your individual care. It is also shared with other organisations to support health and care planning and research.
If you do not want your personally identifiable patient data to be shared outside of your GP practice for purposes except your own care, you can register an opt-out with your GP practice. This is known as a Type 1 Opt-out.
Type 1 Opt-outs may be discontinued in the future. If this happens then they may be turned into a National Data Opt-out. Your GP practice will tell you if this is going to happen and if you need to do anything.

You can use the form above to:
Register a Type 1 Opt-out, for yourself or for a dependent (if you are the parent or legal guardian of the patient) (to Opt-out)

Withdraw an existing Type 1 Opt-out, for yourself or a dependent (if you are the parent or legal guardian of the patient) if you have changed your preference (Opt-in)

This decision will not affect individual care and you can change your choice at any time, using this form. This form, once completed, should be sent to your GP practice by email or post.

 

UPDATE ON OPENING TIMES

As of the 16th December 2019 the phone lines and doors to the surgery will be open from 8am.


TEXT MESSAGES REPLACING LETTERS

We have started to send text messages out to patients instead of sending letters out, this will include us sending a text message to patients to book in for an annual review around the month of their birthdays and various other messages as well.

Please ensure we have your up to date mobile numbers and if you would not like us to send you a text, please let one of our reception team know so we can update your records as declined to receive texts.


UPDATE ON HOW TO ORDER MEDICATION

From Tuesday 30th June 2020, we are no longer taking medication requests over the telephone as we have been during the COVID period.

  • Drop your medication request in the box at reception

Your computerised medical records
New options via PatientAccess 

As part of our plans to develop online services we have now introduced a new service for patients who want to access their records online.

If you are a patient who is currently signed up to PatientAccess to allow you to order your repeat prescription and make your doctors appointments online there is some exciting news. You can now view ALL your current medication items, any allergies or sensitivities and any immunisations that have been recorded on your computer record.

All you need to do to view these additional sections of your medical record is to ask us to activate that part of your computer record for you to see. We will ask you to sign and date a form and then access to these three sections will be available to you within five working days. Once you have signed up for access please let us know if you spot any errors or inaccuracies in your records.

If you do not currently have Patient Access and want to be able to book appointments, order repeat medications and view sections of your computerised medical record, please request this access from reception.
